Require sign-off only where the action can't be undone — sending, publishing, deleting, paying. One gate at the right spot beats ten gates everywhere. And review the artifact, not the process. Auditing forty steps of reasoning invites skimming. Reviewing a final diff, a drafted email, a proposed schedule — that's a task a person can still do well at 4pm on a Friday. Design for the tired reviewer. That's who's actually on shift.
